When enrolling your child at Lockwood Primary School, you must provide us with th following:

- A completed Enrolment form.

- A completed Pupil Information Sheet & completed consent forms.

- A photocopy of a bith certificate/extract.

- An Immunisation Certificate or a completed Statutory Declaration.

- If transferring from another school - a transfer note, any relevant information from the previous school.
